release of information forms
which relaease of info form do I need to obtain for a drug rehabilitation facility regarding a potential resident who is currently in custody in a county jail facility but cannot be released to the drug program with out having sighned this form?

Most of the drug rehab programs have a person that can go to the jail and have him sign the forms. If the rehab facility does not have such a program, they should have the required forms.
