name change on birth certificate

Originally last named Ochoa but father was never in the picture at age 4 father signed over rights and my moms new husband adopted me. I am now 26 yrs old and have always lived with my adoption father's last name. Soc Sec card, License, School, Passport, etc... but when I went to renew my passport as an adult I went to City Records and was told that my birth certificate still has Ochoa. Can I change my birth certificate or do I have to live with that forever? or does that mean my last name wasn't changed legally?

It sounds as if the name was either not changed, or that it was not completed. At this point without extensive court research, you may not be able to find the answer.
