my boyfriend ex wife sent her son to live with us she is going to give up all her parental right to him and give them to me and his dad what forms do we need to file

It's really pretty simple. Your boyfriend (I assume) is the child's father. Assuming the child's mother and father are divorced and the mother has custody pursuant to the divorce judgment, the will need to a Motion, Affidavit and Supplemental Judgment Modifying the custody provision of the original divorce judgment so as to transfer legal custody from mother to father.

Next, if you want to step into the shoes of the mother and permanently take over that role, you will need to adopt the child. Again, this is really easy to do. You will need the following forms:

Petition for Adoption.

Motion for Order Directing Issuance of

Citation.

Order Directing Issuance of Citation.

Mother�s Consent to Adoption. (As required by ORS 109.312.)

UCCJA Affidavit. (As required by ORS 109.309(2).)

Statement of Names and Addresses. (As required by ORS 109.309(5)(a)(A).)

Financial Disclosure Statement. (As required by ORS 109.311.)

Adoption Report form. (As required by ORS 109.400.)

Notice to Grandparents re Adoption. (As required by ORS 109.309(6)(b)(B).)

Affidavit Re Service of Notice on Grandparents.

Verification of Voluntary Adoption Registry Information. (As required by ORS 109.353.)

Request for Waiver of Homestudy and Placement Report

Request for Oregon Criminal History Information
