court order states from years 1 to 5

Does this imean that after age 5 there is no court order since we have never gone back to court for any type of modification and our child is 13 now?

Custody orders are generally active until there is a further order of the court. Even if the court order was anticipated to be in affect until the child reaches age 5 it would still be the order for custody. It would appear that you have been operating without a court order. Either maintain the current custody and visitation or go to court and get a new court order.
