Obtaining Full Custody

I am not married but I have a 5 yr old son. His mother is an absentee parent for the most. Yet we share joint custody. Although I am not named on the birth certificate. His last name is hers. I want full custody of my son and his name changed. He has been with me since birth. How do I do this?
