Legal Question in Family Law in California

What Are The Requirements For An Annulment?

I am inquiring about obtaining an annulment in the state of California. My wife has not made legal changes from her maiden name to my last name. What are the requirements and what forms will be needed if filing for a annulment and how long will this procedure take to complete?

Thank you for your time and cooperation in this matter.
