Legal Question in Family Law in California

HOW DO YOU SUBPOENA FOR A TELEPHONE TESTIMONY IN CALIFORNIA? I need to subpoena 3 people for a court hearing on December 14th. They are all three expert witnesses and I have already asked the Judge for permission to have them testify by telephone since I live in Oregon and the case is in California. Though all three are willing to testify I have to subpoena them as part of their work policy. The options are for them to appear in person (a) or just submit documentations (b) but I need both. I need them to be ordered to appear via telephone and send in documents that will be used in their testimony. Which of the two boxes do I check? Im using form SUBP-002 Civil subpoena. Thank you so much for help! I need to get his done asap!!

You need form SUBP-001, and the clerk of the court will have to sign it and issue it. You must have it personally served in Oregon by someone over 18 years of age not a party to the case and obtain a proof of service. Good luck!
