Legal Question in Civil Litigation in California

When filing a claim against the County (Government), I know that I have to file a Government Claim (form/letter), within 6 months,.... But do I have to use their county's version Claim form that they have available from their county's website/webpage, or can I use my own personally made/hand written version? ...or can I use both, by using their standard claim form and attach my homemade version as an extra attachment?

It does not have to be on their form but it must comply with the requirements of Government Code section 910. If you do not have the right information and did not use their form, you may end up in a situation where you get stuck with arguing whether or not you substantially complied.

The best choice is to use their form and then attach anything else you think relevant.
