What form do I need, in California (Contra Costa County), to request that my small claims case is heard by a different judge/commissioner?

There's no form in Contra Costa County, but you can adapt the one from LA County to meet your needs: http://www.lasuperiorcourt.org/forms/pdf/CIV015.pdf . You must file it with the court at least five days before the first hearing with the judge or commissioner, but the sooner the better.
