Legal Question in Health Care Law in California

My husband and I are 70 years old and trying to understand, in LAYMEN terms, what we need to protect ourselves in the event of a catastrophic medical event. We want to have the directives ready.

Issues such as life support and/or its removal; who needs a copy, how to write the directive, and anything else we would need.

We live in southern CA.
