This question requires more than just a simple answer. First of all, is your friend a US citizen? If she only has a green card then her husband won't be able to get any work permit until years from now.

If she's a citizen, then it depends on whether or not the husband entered into the U.S. illegally. If he did, then he cannot do the paperwork here but must go back to his country and do it there. If he entered with inspection, then he can do the paperwork here and get a work permit soon.

If the person seeking to become a citizen was not initially here legally, then yes, they will have to leave the country, apply through an embassy or consulate, and then wait until the application is approved.
