third person affidavit my information

I am writing a 3rd person affidavit for my brother in law, whose case is AOS pending. I included my name, my current address, my current phone. However,

I was told that I must also include my social security number, my country of birth, and my current immigration status. Is that true?

No, that information is not required. Name, address and contact information is enough.
