Do you have any examples of witness statements that I could get an idea of what the form should look like?

A witness statement is what a police officer takes at an accident scene and deals with bare facts. You probably mean a Declaration which can perhaps be introduced into evidence. if it is the latter, then you need to put it on paper with the proper caption and margin lines and numbers, and then have something like:

I declare under penalty of perjury as follows:

1. My name is _____; I live, phone number

relate why the witness has knowledge, what the knowledge is, objectively connect the facts with what you want to prove thru this person without making it sound like you are arguing the case [not easy for most non-attorneys]

I, , declare under penalty of perjury and the laws of the State of California that the above is true and correct of my own knowledge or belief and that if called as a witness in this matter i could and would so honestly and competently testify. Signed this day of 2011, at California.
