I want to ask a roommate to leave before lease is up

my roommate bounced her check last month for her share of the rent check

this month i was notified she hasn't paid at all

i was also told by the landlord they served us with a 3 day notice on friday the 10th while i was on vacation. the roommate has hidden this from me.

i want to ask her to leave and sign something stating she will leave and that she can be removed from the lease and she will leave on a specific day.

what legal verbiage would I need to include?

what will the landlord need to have on this document so that they can release her from the lease?

Typically a landlord will have a roomate change form. Best you speak with landlord.
