Just go to the court and ask the clerk for the forms. They usually have them there or they will refer you to the County Law Library where they will be available or can be downloaded from a computer and printed for you to fill out. It is SC-100 if you know how to find it online. One place to find it is at the website of the California Judicial Council. I think by just searching in Google for SC-100 it might pop up in some website.
