Legal Question in Real Estate Law in California

The proper document

MY Father is disabled, i now care for him folling my sisters death. I am power of attorney for my father, but i need assistance obtaining the correct doc. for voice for/as my father in case of his death, as far as his property is concerned.

I was told by the court rec. office it is some sort of deed form. what kind? i am not seeking ownership, monetary value or anything. now my siblings/fam. members??? I need to know the proper steps to take to begin this process before it is to late. my dad has wishes he wants me to follow out. Again i want to only be included on the deed with my father as voice. Are there any other steps to be taken besides filing the correct form with the court recorders office?

Is your father mentally competent or physically disabled. If the former, you have to become appointed conservator of his body and/or his estate through bringing a conservatorship proceeding, if the current documents you have from your father do not allow you to make the property and healthcare decisions for him.
