I just want to change the executor of our trust. What do I have to do? fill out?

You need to formally amend the trust. It's a good idea to hire a lawyer to do the amendment because as shown by your question, it is easy to get the terminology wrong when you DIY (you probably mean successor Trustee. An executor is named in a Will.)
