Can I create a last will and testament for my grandmother whom has alzheimer's and is a nursing home?

Not unless you are appointed Conservator of her estate and petition the court for substituted judgment.

If you are her only heir the California statutes of intestate succession provide that you get her estate anyway, even without a will. You will have to probate her estate and ask for letters of administration, however.
