My parents are on social sercuity and SSI they own a home will the home go to the state or county when they are deseased or will it come to me if so should I be on the house deed now for tax propose

it should go to you. have them talk to an estate planning attorney to set up a trust.

It is a good idea to get that living trust going but you should do it right away as there is a two year look-back period for Medi-Cal. If Medi-Cal wants to they can look back two years and claim that any deeds transferred within that time were fraudulently conveyed to avoid paying Medi-Cal. So do it right away, the sooner the better.
