Need help with paperwork re:father's death

Is there any place on the web where I can look for assistance in filling out an ''Affidavit-death of joint tenant'' form? I know it looks pretty straight-forward but I'm just not sure what to put on every single line. I don't want to make a mistake and I can't afford an attorney. thank you for your time.

I think the Los Angeles County Recorder/Registrar's website has one--check their site, or give them a call. A stationery store or your library may also have the form.
