Resuscitation

How do you file a non-resuscitation order for the County of San Bernardino in the State of California? I have an elder parent who has requested this.

Re: Resuscitation

The "Do Not Rescusitate" order should be given to the local paramedics/ambulance company that services their area. In addition, the durable power of attorney for health care (available at the local hospital) can be given to their doctor and save a copy at home.
