Stock Transfers to a Living Trust

I am the sole owner of a corporation and would like to transfer my stocks to my living trust. Is completing the transfer form on the back of a stock certifcate sufficient? Is there a reporting requirement to the State of California?

I have also limited partnership shares. How can I transfer those shares to my living trust?
