termination of guardianship

I need to file an ex parte to terminate temporary guardianship that my mom has over my son. She is in the hospital and will not be able to take care of him any longer. My mom might not agree to willingly terminate the guardianship so therefore this might not be easy. What I need to know is what ex-parte do i need to file as there is more then one.

Is it the ex-parte worksheet and then I attach a declaration to it along with an other form??

Please, please if anyone can tell me what the forms are, I cannot afford an attorney and need to due this all on my own.

Also will I have to serve her with the paperwork as well?

The other thing is that we have a pretrial hearing scheduled for July 6th and the trial set for the 10th I would like to file the ex-parte before that!!

If anyone can just point me to the right direction I would greatly appreciated!

To start off with, I would tell you that if you really cannot afford an attorney, there is a legal aid office in Anaheim (off of Katella) that might be willing to assist you. With regards to which ex parte form you should file.... I don't know that off the top of my head, and you might be better served posting this question to the family law forum as well.
