i live in utah and my sister lives in colo which form would i use i have a matter where my children will be with her for awhile

It depends on whether you want to give you sister a temporary power of attorney which would be fine for a few weeks. If the children will be there for long time, she may need a guardianship, which requires a court action in Utah.
