How do I obtain a prenuptial agreement document that would pertain to a marriage in the Ukraine?

Any prenuptial agreement must be signed PRIOR to marriage, and must conform to the laws of the country where marriage takes place.

So if for some reason you'd decide to marry in Ukraine (and if Ukraine allows prenuptial agreements), then it should meet the requirements of Ukrainian law. Obvioulsy, you will need a lawyer and shall not attempt to use any "samples" available online.

It's always better to marry and sign prenup in the United States, if this is your country of citizenship and residence.
