Legal Question in Civil Rights Law in Connecticut

My employer recently required my colleagues and me to complete and sign a connsent form allowing the State of Connecticut Department of Children and Families to release any information on us that they might have. However, the form also included space for the names, social security numbers, dates of birth, and signatures of any other adults living in the home (past and present) and any children currently living in the home). Is it legal for my employer to require me to disclose private information about other members of my household-- past or present? A colleague of mine was supposedly told, ''Your job depends on it.'' Isn't this a violation of the right to privacy?
