garnishment -- searching for legal form affidavit of exemption

I have been looking for a legal form to remove a garnishment from my check. I was told to request for Affidavit of Exemption. Tell me how and where I can find this form.

Before you attempt to complete and submit the Affidavit of Exemption, you would be well advised

to consult with a person knowledgeable in D.C. garnishment law and issues to determine whether your particular garnishment situation actually qualifies as an exception. (The one who told you to request this particular form may not be the kind of person I am talking about.)
