termination of shareholder agreement

after filling out the form, what else do I do ,besides notarizing it ? do I have to file it with state of fl , and who else do i inform, if any ? I am the buyer .

What does the sellor have to do with it ? and with whom?

There is not enough information in the question. Look at the agreement itself. It should say how it is terminated. They are not typically filed with the state.
