I'm the custodial parent and i would like to know what type of form i would need to have the non-custodail parent sign that would insure my sister the rights to take custody of my if i die or become i'll and to ensure that she would not go to live with an unfit parent

The non-custodial parent has the absolute right of custody of the minior child in the even to your death, unless the non-custodial parent's rights are taken away by the State. Just because you might think (even with good reason) the other parent is unfit doesn't necessarily make it so. The Court must declare the person unfit and terminate the parental rights.
