Custody/Visitation rights

I live in NYC with my husband who has a 4yr old daughter in Orlando, FL. He has had constant communication and even flies down to Florida several times a year (he went there recently in August). Over the past few weeks, his child's mother has had her phones disconnected and has not been responding to his calls when he tries to reach her at her mother's house. My husband wants to go through the courts to get visitation rights but he isn't familiar with Florida family laws. He also wants to be added to the child's birth certificate and acknowledge paternity by signing the paternity form since he hasn't done that either.

How should he precede?

He certainly should contact an experienced attorney in Orlando, since that is where his daughter lives.
