You were certainly not clear at all in your request. Please keep in mind that this is a forum for lawyers to answer some legal questions but not to provide free legal services. If you need a letter, you should go to your local law library and see if they have any books with sample of what you seek. If not, engage the services of an attorney and ask the attorney to draft one for you.

If you meant Letters of Guardianship, they are issued by a court to the Guardian upon the proper filing of a Petition for Guardianship which has been granted by the Court. You will need an attorney to work with you on a guardianship matter as it is a fairly complex matter.
