Relinquishing Parental Rights

Is there a specific legal form for this which a father of a child can use in the state of florida?

If you want to relinquish your rights so that a stepfather can adopt the child, you can use Form 12.981 (a)(1) found at:

If there is not an adoption taking place, then you will either have to draft your own form or hire an attorney to draft it for you.
