name and ssn change

i was adopted when i was 12 and i am 34 now and would like to change my name back to my birth name and change my ssn if possible

You can change your name by filing a Petition for Change of Name. You will need to have a set of fingerprints filed with it which you can obtain at your local police or sheriff's department on their official fingerprint card. Some departments require you to purchase the card at an office supply store and bring it with you.

The petition can be found online if you search google. After filing, have the case set down for a final hearing. I do not believe that you can change your social security number but you can have your new name associated with it.
