Step parent adoption FL

My son's biological father signed a ''Conset Waiver by Parent'' relinguishing his parental rights so that my now husband can adopt him. On the FL court forms it has ''Respondent'' name that I need to enter. Do I have to enter my son's bio father? Even though there?

Do you have the correct form? The case "style" (heading) in adoption cases usually reads: IN RE: THE ADOPTION OF (NAME OF CHILD). While the adoptive parent is the "Petitioner" their name(s) will not appear in the style of the the case.
