if I am off on maternity leave, and have complications and need further time off what are the rules of which I can apply for more time off?

Lot of issues...is your employer large enough that it offers FMLA? How many weeks? Sometimes your employer may offer different types of leaves you can tack onto FMLA, sometimes you can use short term disability. You need to speak to your HR people.
