Legal Question in Medical Leave in Florida

Terminated while on FMLA

I was on FMLA/STD due to a thumb injury. (Not related to work) on 7/14/08. Well I went to the doctor's and was told I can return to work with some resctictions, due to my restrictions, my employer told me I can not return to work. So they put me on STD/FMLA. Well a couple weeks go by and they say I need more evidence on why I need to be out, so I request it from my doctor's office on 9/12/08, and they said it was not enough. So I tried to go back to work, but they said I couldn't without a letter from the doctor saying I can return full duty. Well my next appointment was on 10/2/08 and that was when I was going to get the letter, but I received a letter from HR from my employer and it said I was terminated on 10/01/08 . And the reason for it being I failed to turn in paperwork to them. I was wondering if I would have a case or not if I decided to get an attorney? Also, may I add, I filed for unemployment and was denied because they appealed and said I voluntary quit the job. Can you please help me? Thank you.
