physicians contract agreement
What form can we use to write an agreement between two doctors? We would like to have one pay the other rent for using office space, employees, supplies, etc.

This is not a subject that you can use a form agreement for. You have many state and federal regulations which need to be dealt with pertaining to issues of fee splitting and referral. You need to retain an attorney with knowledge of heath care law.
