tubal liagation done incorrectly

had tubal liagation performed on 4/28/05 1 week later Dr. called and stated that the tubal was not done correctly, i went to the hospital and pulled my medical file the same day she called and it stated everything went good without any problems. when i went back in to have my postpartum check up she stated to my fiance that would perform a vas. on him free of charge and then she prescribed me some birth control pills, so what i'm tring to figure out is what did she do when she cut me open.

Ask her. seriously -- you need to ask your doctor what went on and why the record of the operation says something different. if you don't get a prompt answer, make the request in writing.
