In order to have your name changed, legally, you must be 18 or older or have parental approval. It is then a matter of filing a petition with the Superior Civil Court of your county of residence. You are then required to publish your request in the paper and finally appear in court. The process is generally simple. It can be done individually or with an attorney's help. You can not change your name if you are attempting to defraud creditors.
