Changing my 6 yr olds name to mine

Me and my childs mother arent together anymore and when he was born i wasnt able to get his name to be mine for medicad reasons.What do i need to do to change his name

I am assuming you have some sort of legal custody of the child. If so, you can file a petition to change his name. His mom will have to consent unless it has more than 5 years without contact or support. If she consents, there should be no problems. If she does not, you will have to convince the judge to change the name in a contested case.

See a lawyer (we handle such matters). You do not have an automatic right to change the name. Unless the mother agrees, a Judge will decide.
