Modification of Parental Visitation

How do I go about seeking modification of visitation rights my ex-husband currently has regarding our 11-year old son? There has been so much trouble regarding my ex's behavior and documented drug usage resulting in 2 DFAC's investigations of him where he was found to have emotional abuse in regards to our son. What can be done to restrict his visitation rights as originally stated in our divorce decree? Can he be required to seek professional help?
