Re: need to file a personal injury lawsuit

The only form that is available is in Magistrate Court, which only has jurisdicition up to $15,000.00. However, you can use this form as a model to draft a complaint to be filed in State Court or Superior Court. Do this ASAP. Once you do that, consult with a PI attorney ASAP.

Re: need to file a personal injury lawsuit

I'd suggest getting on the phone this evening to people who may have used a local personal injury attorney, or know a lawyer who could recommend a personal injury attorney. Do what it takes to get an appointment first thing in the morning, and have every document prepared to give to the attorney. Perhaps a couple PI lawyers will respond here. This isn't the best way to find a lawyer, and you should have started long ago, but at this point you are better off doing this than trying to find a "form" to file or expect a lawyer to guide you through the process via a forum like this. As with all lawyers, read and understand any fee agreement you sign.
