Legal Question in Wills and Trusts in Georgia

affidavits

I need affidavits for the following:

#1- That no conservator has been appointed for the minor's estate.

#2- The value will not exceed $ 15,000 in value.

#3- That I am the natural guardian of the children.

I am requesting that my childrens benefits be transfered into a bank account so that is not controlled by the life insurance company and these are the following things that's being requested. My question is do I need legal counsel for this matter. I have already called the courts and I was told that in the state of GA I didnt need to have a court document stating I am the guardian , which I am the birth parent of each child. as long as it was not over $ 15,000 I just need some advise on this matter

Re: affidavits

Basically, this is it but you may need to modify it a bit:

AFFIDAVIT

#1- That no conservator has been appointed for the minor's estate.

#2- The value will not exceed $ 15,000 in value.

#3- That I am the natural guardian of the children.
