Photo Release Form?

I take photos at numerous local race tracks and sell my photos to the racers through my website. Recently I was asked for 6 various photos and images to be used in a calendar that the track plans to sell. Is their a form that I can get the person that wants my photos, to sign that says the photos or images I am supplying are to used for the calendar and may not be reproduced for anything else? And also states that I the owner of these images must approve any lab work that is done to my original photos and images before the calendar is printed. Is this something I just need to type out and have them sign with the header ''Photo Release'' and or ''Terms and Conditions''?

Yes, what you need is a contract drafted to cover the rights and obligations of the parties and cover the legalities involve so that you do not have to face litigation down the road.

Any copyright lawyer in your area should be able to provide you with the appropriate license form. Be sure that you have registered the images with the US Copyright Office before you license them. Again, a copyright lawyer in your area should be able to help you do this.
