Legal Question in Administrative Law in Illinois

name change

When I got divorced - my papers stated that I could go back to my maiden name if I so desired. At the time, I did not do it. (six years ago) However, I want to do it now - must I go through the legal channels or can I just use my divorce papers? What is my next step?
