Legal Question in Civil Litigation in Illinois

I need to know how to file a motion with the court of Appeals.and what do I need to say?Can I down load a motion? Do I need to state the grounds for Appeal?

I had a civil matter pending before the Central District Court in front of the chief Judge Michael McCuskey "he also was the one who Dismiss my case" I file an Appeal and dismiissed and was dismissedt on the grounds of GOOD FAITH. I really don't know to much about law but I do know that I want to fight this till the end. I was really substain a BACK INJURY and was really hurt. I reaaly din't know anything about law and since this happe while in prison my capebalities were limited PLEASE HELP THANK U AND GOD BLESS. CV -10-2259

You need to consult with an attorney as soon as possible. If you cannot find one, look for a free legal clinic in your area. There are many deadline and requirements that have to be met to file an appeal. If you do not comply with them, your case may never be heard.
