Legal Question in Insurance Law in Illinois

I am ILL. resident. I have private accident disability insurance. I was hurt 2/4/2011. Insurer paid me from 4/20/2011 till 10/31/2011 then cut me off saying my problems were degenerate not accident related. My new Dr. has verified saying the accident has been the cause for my now ongoing disability. My claim was still open if I provided new info. I provided the new info now they want me to sign a blanket form medical release form that I guess would release info clear back to the crib. Other web sites say not to sign this type only sign for what pertains to the case. What do you say? I am 62 years old I was awarded SSDI in 7/2012 Saying I was disabled since 4/20/2011. This is not workmens Comp I had I was self employed at the time and had been at the same Occp. for 41 yrs. Also do you do private ins. law if I need help?
