Legal Question in Employment Law in Illinois

I am 52. Live in Illinois. I had a long career in my profession for 25+ years. In 2008 I got Parkinsons. In 2011 I struggled to perform the material and substantial duties of my chosen career and was let go Jan 4th 2012 (officially my last day worked). My Doctor mistakenly entered January 20, 2012 on the 'first day I could not work' field in the attending physicians statement form I returned to Lincoln Life. They denied my claim and also said this in no longer a group LTD claim (for appeal purposes). Now I am appealing, but I need the claim to be group because the benefit is three times greater. I need to tell Lincoln why the doctor put January 20th, 2012. What can I tell them that put this issue to rest so we can proceed?

Sounds like the MD needs to resubmit a corrected form and then the insurance company will have to agree to allow the resubmittal. You may need help with this so I recommend having an attorney help you.
