Does an employee in the state of Illinois have the right to make a former employee sign a statement before releasing the final paycheck? My understanding is that the paycheck must be sent on the next scheduled pay date and an employee has no obligation to sign a form stating they quit.

I believe you are asking if the "employer" can make you sign a "statement" before releasing the final paycheck. You don't mention what the statement is but generally an employer cannot condition release of a paycheck upon singing a "statement."
