Legal Question in Real Estate Law in Illinois

I live in a housing owned house, in Lasalle county, IL>. My home has terrible, old windows, mold, rotting floors and constant leaking in sink. My bathtub is old, ,moldy and does not have proper grouting. Where can I go to get action? My manager will not listen to me. I am on disability, if that matters.
